I certify that the information that I supplied in the client questionnaire, home care maintenance, and deposit agreement are correct I agree to indemnify and hold harmless the Fringe Parlor, Sunny and Shears, and/or its employees for lost damage, claims or actions of every kind that may suffer as a consequence of my failure to provide them with accurate and complete information. Home Care Maintenance for Extensions Clients HOME MAINTENANCE FOR KERATIN BONDED EXTENSIONS 1. Do not shampoo hair for 2 days after receiving extensions in your hair. The bonds need to be cured. Shampoo or excessive pulling on hair will result in loosening the bond. 2. Hair should be braided or put in a low ponytail when going to bed. This will prevent pulling & matting of the hair while you sleep. 3. Never ever go to bed with wet hair! This will mat & damage the hair. Both natural hair and hair extensions are weakest when wet. To avoid breakage, apply a detangler, leave-in conditioner & make sure the bond is dried. 4. Always use gentle strokes when brushing the hair. Pulling on hair will result in breakage. Hold at the bond, & brush under if getting tangles out. 5. Never use any product containing silicone, glycerin, lanolin, sulfur (dandruff shampoo), or any kind of oil. These products will break down the bond. 6. Go through your hair everyday with your fingers to make sure your bonds are not getting tangled. Very gently run your fingertips through the hair. 7. Shampoo hair by leaning your head backwards to prevent tangling. 8. Gently massage scalp & hair when shampooing. 9. Always make sure that hair is dry around the bond area, you can let the rest air dry. 10. When styling with any heat tools, be very careful not to apply direct heat to the bond & use medium heat when drying. Direct heat will melt & soften the bonds. 11. It is okay to swim. It is recommended to wear a cap, chlorine & salt can cause hair to tangle, mat, or break. Always shampoo with a PH balancing shampoo & dry hair afterwards. 12. Activities taking place in a constant damp environment such as aerobics, steam baths/saunas, or hot yoga, may lessen the longevity of the Keratin Extension service. 13. When exercising or swimming, it is recommended to wear hair up or in a ponytail to prevent tangling or damage. 14. Never perform your own chemical service on your extensions. Always have your stylist assist in any services. Some extension loss is normal and to be expected. Average client hair loss is 50-150 hairs a day. A full head application covers approximately two-thirds of the head. You can expect to see a small quantity of naturally released hairs trapped in the attachments after some time. This is normal and should not be interpreted as hairs pulled out of the scalp by the extensions themselves. Daily brushing close to the scalp and finger separation of the applied strand will avoid matting in this area. Pull trapped hairs back toward the scalp to loosen before separating strands. -Make sure to have a follow-up appointment two weeks- one month later with your stylist so they can check on your extensions. VERY IMPORTANT Excess shedding hair & extension loss has been known to happen with the following: *Starting a new medication or changing dose on existing meds *Soon after child birth or up to 6 months after breast feeding *Shortly after having covid & possibly lasting up to 4-6 months *After covid vaccines or boosters *Experiencing extreme stress PLEASE contact your stylist if you notice an abnormal amount of shedding or extension hair loss Consent(Required) I read, agree with, & submitted my Maintenance Sheet. Extensions Deposit Form EXTENSION DEPOSIT I understand and agree that a deposit is required to make an appointment for an extension installation. This deposit will vary in price due to desired look, type of extension and amount of work going into service. Deposits are NON REFUNDABLE. Hair is custom ordered to each specific individual. Hair cannot be returned or exchanged once order is placed. A receipt will be emailed to you after deposit is made with your personalized quote, deposit made, and amount remaining. If hair is not purchased through one of our distributors,& is brought to us from outside our establishment, & stylist agrees to install. Salon & stylist will not guarantee longevity or quality of extension service. Additionally you agree to notify The Fringe Parlor no less than 5 business days if you change the color or the texture of your hair between the consultation and the application appointment. If you change the color or texture of your hair during this time you will be required to book a new consultation appointment and a new deposit will be required. If you cancel less than 48 hours prior to appointment, you will be charged a $100 to book another appointment. If you NO SHOW NO CALL the day/time of your service, you be charged 50% of the service total. This will NOT go towards a future appointment. You will still be responsible for the entire amount specified on agreement to have service performed. I understand and agree extension removal service is $75 an hour (keratin & tapes) unless specified otherwise. I understand that keratin bonded hair may NOT be reused. Tapes & wefts may be reused until the integrity of the hair is no longer to our standards. Home Maintenance - Hand Tied Weft & Tape-in Extensions Home Maintenance - Hand Tied Weft & Tape-in Extensions Hair extensions last six to eight weeks before needing a touch-up. application time is fast—30 minutes to an hour for tape-in and 1.5 - 2 hours for hand tied weft. After eight weeks, you’ll start to feel pressure and irritation on your scalp. The longer you leave them in, the higher chance of hair breakage. A proper extension brush from your stylist will help keep your extensions in check Brushing Both natural hair and hair extensions are weakest when wet. To avoid breakage, apply a detangler, leave-in conditioner, or oil to the hair before brushing. Always use a hair extension brush to avoid tugging on the tape. The proper way to brush is to start from the bottom, working your way up. If you start from top to bottom, you will cause tangles and breakage, just like with your natural hair. Also, remember to pat dry the hair extensions when using a towel. Rubbing a towel back and forth can cause a matted mess because of the increased friction. Whenever possible, blow dryers should be avoided; leave the extensions to air dry. Constant heat will damage the hair faster. When blow drying Never blow-dry upside down. it is rough on your extension & can cause damage and tangling. When Shampooing Your tape-in hair extensions should always be washed and conditioned with sulfate-free products. Any products containing sulfates can dry up the hair extensions. Conditioner should be applied only at the mid-shaft through the ends. If any oils or conditioners are placed on the tape or scalp, they will weigh down the extensions, just as they would your natural hair. - Brush your dry hair. This will make it easier to section and shampoo when you shower. - Wet and section your hair. - Put shampoo on your fingertips and gently glide over the scalp and across the top of your extensions. - Rinse and repeat with the next section. - When blow drying never blow-dry upside down. it is rough on your extension & can cause damage and tangling. - Sleep with your hair in a braid or low ponytail. This method keeps your hair extensions from moving and provides that extra bit of comfort. Color Correction/Project Form COLOR CORRECTION SERVICE I verify that I verbally consulted with my stylist, and discussed the services that I will need in order to achieve my desired color. I give permission to my stylist to perform service(s) and I will not hold her liable for any potential imperfections and/or potential damage from the service(s). I am aware that color correction may or may not take MULTIPLE sessions spaced anywhere from 2 to 6 weeks apart to achieve desired healthy result. If I am displeased with END result of said correction process, I agree to immediately contact my stylist no more than 1 week after the application. I understand that any complaint after 1 week will still be resolved, however, I will pay the full service price. I acknowledge my stylist’s aftercare and product recommendations to help maintain my desired color. I acknowledge if I DO NOT purchase aftercare recommendations, the Fringe parlor or its stylists will not be held liable for longevity of color or health of hair and will not guarantee results.
